Pantoprazole sodium sesquihydrate 22.57mg (20mg as Pantoprazole)
1. Concomitant antibiotic therapy to prevent recurrence of gastric and duodenal ulcers infected by Helicobacter Pylori
- For adults, administer 40mg pantoprazole twice a day (morning and evening, before meals) for 1 week without chewing or crushing with the following antibiotics
(1) Either 1,000mg amoxicillin and 500mg clarithromycin twice a day for one week, or;
(2) 500mg metronidazole and 500mg clarithromycin twice a day for one week, or;
(3) 1,000mg amoxicillin and 500mg metronidazole twice a day for one week.
- Use the metronidazole therapy of (2) and (3) only when (1) is inappropriate.
- If additional therapy with pantoprazole is instructed to ensure proper treatment of ulcers, the following dose recommendations according to the following method for duodenal and gastric ulcers should be considered
- If concomitant therapy is not necessary (e.g. negative for helicobacter pylori infection), carry out pantoprazole single therapy according to the following method.
2. Gastric ulcers and reflux esophagitis
- For adults, take 40mg pantoprazole once a day in the morning without meals without chewing or crushing.
- Healing usually occurs within 4 weeks, but healing may take longer depending on the patient. If healing does not occur after first 4 weeks of administration, continue administration for 4 more weeks.
- If patient was nonresponsive to other therapies, dose may be doubled (80mg pantoprazole daily).
3. Duodenal ulcers
- For adults, take 40mg pantoprazole once a day in the morning without meals without chewing or crushing.
- Healing usually occurs within 2 weeks, but healing may take longer depending on the patient. If healing does not occur after first 2 weeks of administration, continue administration for 4 more weeks.
- If patient was nonresponsive to other therapies, dose may be doubled (80mg pantoprazole daily).
4. Zollinger-Ellison syndrome and other pathological states of excessive gastric secretion
- After beginning with 80mg pantoprazole daily, increase or reduce depending on gastric secretion measurements.
- For doses exceeding 80mg, administer over two times daily.
- Temporary increases in dosage to 160mg or higher are possible, but only enough to adequately regulate acid.
- No limit on duration of therapy, but must be adjusted depending on clinical needs.
ㆍReduce dose for patients with severe liver impairment, administering 40mg pantoprazole once every other day.
ㆍFor elderly patients or patients with renal impairment, do not exceed 40mg pantoprazole per day. For elderly patients, 40mg of this drug may be administered twice daily (before meals in morning and evening) without chewing or crushing for one week as part of the above concomitant therapies with antibiotics.
ㆍConcomitant therapy with antibiotics must not be used for moderately severe to severe liver/renal impairment patients. Insufficient data on efficacy and safety of such concomitant therapies.
